Hybrid role but does have some member visit requirements - Must reside in Cuyahoga County or Lake/Geauga County and be able to commute to in-person visits

Shift (Start/End Time): 8am-5pm Mon-Fri
Pay Rate: $38 - $48 hourly plus travel reimbursement

Daily Responsibilities:

• Face to Face with members in Long Term Care and Assisted Living Facilities – assessments, Care Plans, review of medications, and any needs.

Performance Expectations/Metrics:

• 55-85 touches per week.

What previous job titles or background work will in this role?

• Case Manager, Home Health Nurse experience preferred

Required Skills/Experience:

• RN Licensure in GOOD standing in Ohio
• Must reside in Cuyahoga County or Lake/Geauga County and have a Valid Driver’s license
• Experience in Care Management
• Long term Care, Home Care, and/or MDS
• At least basic Microsoft office and electronic medical record experience

Preferred Qualifications:

• Home Care background- where in person visits were completed
• Waiver knowledge preferred
