RN CASE MANAGER - HOME HEALTH | DETAILS AND COMPENSATION:
- Location: Grand Junction CO 81506 – 100% On-site,
- Payrate: $56.14/hr Mileage Reimbursement: $0.43/mile Field Work
- Coverage: Mesa County
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM EVERY OTHER WEEKEND
RN CASE MANAGER - HOME HEALTH | SUMMARY AND HIGHLIGHTS:
- Perform comprehensive assessment of high-risk members.
- Collaborate with primary care providers to implement individualized care plans.
- Continuously reassess cost-efficient, appropriate levels of care.
- Coordinate care transitions with physicians, social workers, and claims professionals.
- Engage specialty resources for optimal resolution of patient care issues.
- Keep detailed records of clinical, functional, and fiscal outcomes.
- Act as patient advocate by protecting privacy and confidentiality issues
RN CASE MANAGER - HOME HEALTH | RESPONSIBILITIES:
- Perform comprehensive assessment of hospice patients.
- Collaborate with primary care providers to ensure implementation of individualized care plans.
- Continuously reassess cost-efficient, appropriate levels of care.
- Coordinate care transitions with physicians, social workers, and discharge planners.
- Engage specialty resources for optimal resolution of patient needs.
- Identify opportunities for health promotion and illness prevention.
- Act as patient advocate by protecting privacy and confidentiality.
RN CASE MANAGER - HOME HEALTH | REQUIREMENTS:
- Education: Associate or Bachelor’s degree in Nursing (or related field)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Case Management certification
- Strong ability to analyze complex medical information and make sound clinical decisions
- OASIS and HCHB experience
- Multi-state RN license
- HH EXPERIENCE - IVS, WOUND VACS, OASIS, OSTOMIES, FOLEYS, CHEST TUBES
- Current TB test
- CPR/BLS certification
- Wound care competency
- Valid insurance and ability to travel locally
