Registered Nurse Case Manager Highlights

• The pay for this position is $38.00 per hour
• Monday – Friday, 8:00am – 5:00pm EST (some flexibility available)
• Fully remote, field-based role with local travel up to 75%
• This position is a full-time contract assignment

Responsibilities

• Perform at least 10 nursing facility (NF) member assessments weekly
• Support the Health Plan Rebalancing Initiative to transition nursing home members into the community
• Visit NF members in-person at least twice per week to complete screenings and rebalancing events
• Assess the home environment prior to discharge, identify environmental needs (e.g., DME, ramps), and coordinate services accordingly
• Conduct in-person Significant Change Visits within 5 days of member transition
• Collaborate with facility Business Offices weekly to track census and coordinate with Social Services and Case Management on discharge plans
• Partner with case managers to identify and support high-risk members and implement interventions for safe transitions
• Participate in community relations events (minimum of 2 per year)
• Develop and maintain strong member relationships to enhance satisfaction and retention

Requirements

• Registered Nurse (RN) with an active, unrestricted license in Florida
• Associate Degree in Nursing required; BSN preferred
• Bilingual Spanish/English required
• Minimum 2 years of clinical experience
• Must reside in the Tampa area and be able to travel within Hillsborough, Highlands, Polk, Hardee, and Manatee counties
• Reliable transportation and willingness to travel up to 75% locally
• Experience with Managed Care, Discharge Coordination, Home Health, or Case Management preferred
• Strong computer skills and ability to navigate multiple systems in a fast-paced environment
