HOME HEALTH REGISTERED NURSE | DETAILS AND COMPENSATION:
- Location: Albemarle NC 28001 – 100% On-site

- Covering Counties: Stanly, parts of Davidson, and Montgomery
- Payrate: $55.95/hr + Mileage
- Required Availability: Full-Time | Monday–Friday, 8:00 AM – 5:00 PM

- Visits or points: 5-7 visits per day and/or 6 points per day
HOME HEALTH REGISTERED NURSE | SUMMARY AND HIGHLIGHTS:
The Home Health Registered Nurse will provide community-based nursing care, case management, and clinical oversight for patients across designated counties. This role involves in-home visits, care coordination, medication management, staff supervision, and patient education. The ideal candidate is organized, clinically strong, and comfortable working independently in the field.
- This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
- Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

HOME HEALTH REGISTERED NURSE | RESPONSIBILITIES:
- Assess patient health status and implement individualized care plans
- Provide health maintenance, disease prevention, and case management services
- Observe, assess, plan, implement, and evaluate nursing care for well, chronic, and acutely ill individuals
- Coordinate care and communication with physicians, nurses, and other healthcare professionals
- Ensure proper medication management, administration, storage, and documentation per policy
- Communicate changes in patient condition to physicians and ensure timely progress reporting
- Transcribe and manage physician orders and medically related documentation
- Complete required daily, monthly, quarterly, and annual nursing documentation
- Maintain accurate nursing notes reflecting patient health status
- Supervise licensed practical/vocational nurses as needed
- Train new employees and conduct annual health and safety recertification training
- Ensure unlicensed staff are trained on patient-specific health, safety, and disease prevention needs
- Maintain equipment, supplies, and furnishings to prevent disease transmission
- Coordinate care during hospital admissions and discharges
HOME HEALTH REGISTERED NURSE | REQUIREMENTS:
- Education: High School Diploma or GED (verifiable)
- Attendance: Commitment to mandatory attendance during the first 90 days.
- Active Registered Nurse (RN) license
- Homecare Homebase (HCHB) experience required (must be clearly listed on resume)
- OASIS experience required (assessment administered on day one; onsite training required if not passed)
- Wound care experience required (certification not required)
- IV infusion and PICC line care experience required
- CPR/BLS certification (must be completed onsite; online-only certifications not accepted)
- Valid driver’s license and active insurance
- Ability to work independently in a community-based setting
