Job Description
  • Perform care management duties to assess and coordinate services for high acuity populations.
  • Develop personalized care plans for long-term care members and educate them on available services.
  • Collaborate with care management teams to identify necessary providers and resources.
  • Monitor member status and revise care plans as necessary to meet goals and needs.
  • Collect and maintain member information to comply with state and federal guidelines.
  • Act as a liaison and advocate for members, coordinating community resources and services.
  • Engage in onboarding and training for new hires, providing support and guidance.
Critical Information
  • This position is field-based, requiring visits in Steuben and surrounding counties.
  • Applicants must reside in Steuben County and be able to conduct 2-4 UAS Assessments or Reassessments daily.
  • Must have a valid driver's license and personal transportation preferred, but public transportation will be considered.
Education/Licenses Needed
  • Graduate from an Accredited School of Nursing or a Bachelor's degree in Nursing preferred.
  • NY Licensure as a Registered Nurse (RN) or Nurse Practitioner (NP) required.
  • Minimum of 4 years of relevant RN experience.
