Job Description
  • Perform care management duties to assess and coordinate medical and supporting services for complex/high acuity populations.
  • Develop and continuously assess personalized care plans for long-term care members and collaborate with the care management team.
  • Monitor member status for complications and clinical symptoms, revising care plans as necessary.
  • Educate members and their families on services and benefit options available for high-quality care.
  • Conduct home and/or site visits to assess member needs and collaborate with resources.
  • Collect and document all member information and care management activities to ensure compliance with current guidelines.
Critical Information
  • This is a field-based position requiring residency in Onondaga County to cover surrounding counties.
  • Must be willing to conduct 2-4 UAS Assessments or Reassessments daily, M-F.
Education/Licenses Needed
  • Graduate from an Accredited School of Nursing or a Bachelor's degree.
  • RN - Registered Nurse State Licensure required.
