Senior LTSS Service Care Manager (RN) Position Overview
Seeking a Senior LTSS Service Care Manager (RN) to provide comprehensive care management services to members with complex and high-acuity needs. This field-based role requires conducting in-home and community visits, performing UAS assessments, and coordinating medical and supportive services to ensure members receive appropriate, cost-effective, and quality care.
Senior LTSS Service Care Manager (RN) Key Responsibilities
- Conduct 2–4 UAS Assessments or reassessments daily in members’ homes or community settings.
- Develop, implement, and manage individualized care plans.
- Assess medical, behavioral, and social support needs for members.
- Coordinate services across providers, caregivers, and community resources.
- Educate members and families regarding care plans, treatment options, and available services.
- Collaborate with interdisciplinary care management teams.
- Ensure compliance with state and federal regulations and LTSS program requirements.
- Support quality improvement initiatives and care coordination best practices.
- Serve as a clinical resource and may precept or mentor new clinical staff.
Senior LTSS Service Care Manager (RN) Required Qualifications
- Active New York Registered Nurse (RN) License
- Residency in the Bronx, NY area
- Minimum 4 years of RN experience
- UAS Assessment experience
- Willingness to work in the field Monday–Friday conducting home/site visits
- Ability to perform multiple daily assessments
- Strong assessment, care planning, and coordination skills
- Excellent communication skills for interaction with members, families, and providers
- Graduate of an accredited School of Nursing (Bachelor’s degree preferred)
Senior LTSS Service Care Manager (RN) Preferred Qualifications
- Bachelor of Science in Nursing (BSN)
- Care management experience, preferably in a Managed Care Organization (MCO)
- Experience working with complex or high-acuity populations
- Knowledge of community-based resources
- Leadership experience or ability to mentor clinical staff
Senior LTSS Service Care Manager (RN) Additional Requirements
- Driver’s license required
- Personal transportation preferred (public transportation acceptable)
- No relocation assistance
- No visa sponsorship available
