Service Care Coordinator Compensation

• The pay for this position is $23-$28 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Service Care Coordinator Highlights

• The required availability for this position is Monday – Friday from 8AM – 5PM, possible OT required
• Remote but field visits required to member's homes in Dallas! Must reside in Dallas tx area!

Service Care Coordinator Job Responsibilities:

• Complete Face to Face, Zoom, or Telehealth assessments, up to 4 assessment per day x3 days per week
• Complete required forms for all assessments completed within required time frames, Documentation days x2
• Work assigned tasks/assignments to address member needs/requests
• Work other possible assignments such as Complaints & Escalations
• Possible high priority assignments that will require urgent attention
• Update member record as needed

Describe the performance expectations/metrics for this individual and their team:

• Meet required deadlines, 3 day turnaround time for documentation of any assessment
• Possibility of High priority assessments that must be completed within 24 hours
• Possibility of OT needed to address business needs
• Deliver thorough assessment, clear documentations
• Deliver quality of all work performed
• Available to work M-F 8-5, with possibility of work needed outside of 8-5 schedule
• 75% Travel required to complete assessments, possibility of travel of more than 30 miles to reach or members
• Good communication skills and collaboration with other Teams, Providers involved with member’s care
• Ability to quickly adapt to change and collaborate well with team members
• Ability to identify health and safety risks/gaps, holistically assess, and coordinate member care

Service Care Coordinator Requirements

• LPN or LVN or LSW
• Valid drivers license
• Speak/Write English & Spanish
• Know/Understand Medical Terminology and Disease process
• 2+ years of experience in a managed care environment or working with people with disabilities and vulnerable populations who have chronic or complex conditions

Service Care Coordinator Preferred Qualifications

• Field work experience with the elderly and disabled population
• Experience in clinical setting
