Social Work / Human Services Case Manager
Looking for a Social Work Case Manager -No License Required, Hybrid Remote and in Field working in Wyandotte or Johnson County
Hours: Monday – Friday, 8am-5pm, no weekends, no holidays
Pay: $29.45 + Mileage
Travel in County Required:
- Travel time is paid at the hourly wage
- Mileage is reimbursed at the IRS rate
- Must live in one of the following counties:
- Wyandotte or Johnson County
Social Work Case Manager Job Duties:
- Coordinates case management activities for Medicaid Long Term Care/Comprehensive Program enrollees.
- Conducts comprehensive evaluation of Members using care management tools and information/data review
- Coordinates and implements assigned care plan activities and monitors care plan progress
Conducts multidisciplinary review to achieve optimal outcomes
Social Work Case Manager Requirements:
- Case Management experience required
- Bachelor’s Degree Required (Social Work or related field)
- Must live in one of the listed counties
- Training will be remote for 2 weeks
- Will travel in the county up to 50-75% of the time
Social Work Case Manager Preferred Qualifications:
- Long Term Care experience preferred
- Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
- Effective communication skills, both verbal and written
