A-Line Staffing is now hiring MSW Social Worker in Pompton Plains, NJ. The MSW Social Worker would be working for a Major Healthcare System and has career growth potential.

MSW Social Worker Highlights:

• Schedule: Monday–Friday, 8:30 AM – 4:30 PM (weekends as needed)
• Pay Range: $55-$60 hourly

MSW Social Worker Responsibilities:

• Collaborate with a multidisciplinary care coordination team to support high-risk/high-cost patients
• Conduct psychosocial assessments, social determinants of health (SDOH) screenings, and create individualized care plans
• Provide telephonic/virtual counseling using evidence-based techniques
• Address barriers to care and connect patients to sustainable medical and social resources
• Maintain timely and accurate documentation and intervention records
• Engage with internal care teams and external community resources
• Provide consultative support to community health workers
• Participate in departmental trainings, meetings, and case conferences

Time Allocation:

• 60% – Patient assessment, referrals, interventions
• 20% – Documentation
• 10% – Program development and meetings
• 10% – Interagency collaboration

MSW Social Worker Required Qualifications:

• Acute Inpatient Case Management and Discharge planning.
• Master’s Degree in Social Work from an accredited institution
• Active LSW license in New Jersey, in good standing
• 3–5 years of experience in a health-related setting; hospital experience preferred
• Strong knowledge of social services, insurance, and advocacy systems
• Familiarity with Epic EMR preferred
• Proficient in Microsoft Office and outcome reporting
• Strong interpersonal, organizational, and communication skills
• Compassionate, culturally competent, and able to work with diverse populations
• Bilingual a plus
• Behavioral health experience preferred
