Social Worker Compensation

• Hourly Pay Rate: $26 -$29/hour based on experience
• Benefits are available to full-time employees after 90 days of employment.
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Social Worker Highlights

• Shift Notes: Position Shift: 8a – 4pm (30 min lunch)
• Statewide travel required as necessary including some overnights.

Social Worker Responsibilities

• Conduct interviews with individuals, families, and agency representatives to gather and update psychosocial information. Assess psychosocial strengths, weaknesses, and service needs for eligibility.
• Provide counseling using various modalities, assisting individuals and families in addressing and improving personal and social behavior.
• Complete annual and initial psychosocial assessments, maintain case files, document progress, and record interactions with family members and external contacts.
• Participate in multidisciplinary treatment team meetings, develop care and treatment plans, and ensure necessary benefits and services are provided.
• Apply for benefits like Medical Assistance and Social Security, assist in legal matters, complete mental health petitions, and respond to court-related correspondence.
• Coordinate discharge planning, write discharge summaries, and aid in transitions to community care.
• Work with community-based services, health care agencies, probation officers, and legal entities to facilitate care and compliance with legal requirements.
• Conduct risk assessments, assist with financial transactions, schedule active treatment programming, and maintain contact with relevant legal and healthcare representatives.
• Lead active treatment groups aligned with individualized care plans and participate in developing and administering treatment programs.
• Attend multidisciplinary meetings, testify in hearings, and handle related administrative tasks as assigned by supervisors.
• Transport patients for trial or home visits and conduct inspections of service providers.
• Notify supervisors of unit issues and participate in calls and meetings related to patient care.

Social Worker Requirements

• Possession of a degree in Social Work. Past experience working in the mental health field preferred
