Compensation:

Pay range: $45 - $52 per hour, commensurate with experience.

Your Role:

Coordinate and provide compassionate care for veterans, ensuring they receive the highest standard of healthcare.

Educate patients and families about health conditions, treatments, and preventive care.

Perform essential administrative duties to streamline patient care and improve outcomes.

Collaborate with healthcare professionals to ensure veterans get the best possible care.


Scope of Care:

General:

The RN assigned to these areas provides direct and indirect nursing care by contributing to the patient’s assessment, determining priorities, identifying nursing measures and therapeutic objectives, and evaluating outcomes. This role requires independent judgment, flexibility, organizational skills, and an interdisciplinary approach to patient care.


Organizational Relationship:

The RN reports to the VA Nurse Manager or designee during the workweek.


Duties and Responsibilities:

Office of Community Care (OCC) Registered Nurses:

Clinically review consults/requests for completeness and appropriateness/utilization review.

Disposition care in the community per utilization review and guidelines.

Conduct clinical reviews of incoming medical records and communicate findings with ordering providers and PCPs.

Coordinate care by corresponding with veterans, community providers, internal providers/nurses, and administrative staff to ensure proper authorization and processing of care requests.


Referral Coordination Team (RTC) Registered Nurses:

Triage specialty consults for clinical appropriateness and completeness within National RCI Standards.

Communicate with veterans via phone to explain healthcare options, allowing them to make informed decisions about where and how to receive specialty care.

Provide clinical knowledge to veterans and serve as a resource for multidisciplinary staff (MDs, LIPs, SW, HAS, etc.) within and outside VISN19.

Collaborate with CACs to rebuild or modify consult templates.

Serve as a liaison between internal staff and Community Care (CC) staff during the consult process.

Act as a trusted point of contact for ordering providers and specialty care services where RCI is implemented.

Build relationships with specialty providers based on vetted RCT Specialty Triage Tools developed in collaboration with VA specialty providers.


What We’re Looking For:

At least 2 years of experience as a Registered Nurse Case Manager.

Associate’s or Bachelor’s degree in Nursing from an accredited institution.

Active, unrestricted RN license to practice in the U.S.

Active BLS (AHA) certification.

Strong communication skills, with a focus on patient care and education.

Basic computer skills, including proficiency in electronic health records.


Physical Requirements:

Ability to perform job duties in both clinical and office settings.

Ability to communicate effectively with patients and staff.
